Admissions Documents

For all applicants submitting an application to M'KIS, please refer to the Applications Checklist.

Below you will find the relevant documents according to grade level of the prospective student(s).

Applications may be submitted via e-mail, by fax, by mail, or in person.

Elementary School Application

For families with students applying to Elementary School (PreK Age 3 - Grade 5), please submit the following:

  • A completed application form
  • Copies of all academic records such as report cards from this current year and the past three years. (If necessary these must be translated into English and certified). 
  • Copies of results from standardized tests such as MAP, ITBS, ISA, STAAR, STAR. 
  • A photocopy of the passport ID page of the applicant and that of the parents. 
  • A copy of the visa of each applicant and that of the parents is required as soon as available and is a requirement prior to enrollment*. 
  • Two passport size photos. 
  • Health Record Form. 
  • Medical Physicians Form
  • A copy of the immunization records. 
  • KP to G5 Teacher Recommendation
* Please note that for an international student to enroll at M'KIS, they must have a valid visa (such as a dependent pass or MM2H). Additional information regarding student and guardian visas, may be found here.

Middle School Application

For families with students applying to our Middle (Grade Six - Grade Eight), please submit the following:

  • A completed application form
  • Copies of all academic records such as report cards from this current year and the past three years. (If necessary these must be translated into English and certified). 
  • Copies of results from standardized tests such as MAP, ITBS, ISA, STAAR, STAR. 
  • A photocopy of the passport ID page of the applicant and that of the parents. 
  • A copy of the visa of each applicant and that of the parents is required as soon as available and is a requirement prior to enrollment*. 
  • Two passport size photos. 
  • Health Record Form
  • Medical Physicians Form
  • A copy of the immunization records. 
  • MS/HS Recommendations Form.

High School Application

For families with students applying to our High School (Grade Nine - Grade Twelve), please submit the following:

  • A completed application form
  • Copies of all academic records such as report cards from this current year and the past three years. (If necessary these must be translated into English and certified). 
  • Applicants to High School must provide an official, signed and stamped transcript of all high course work and credits earned in a sealed envelope to be given to the school upon arrival. 
  • Copies of results from standardized tests such as MAP, ITBS/ITED, ISA, STAAR, STAR. 
  • A photocopy of the passport ID page of the applicant and that of the parents. 
  • A copy of the visa of each applicant and that of the parents is required as soon as available and is a requirement prior to enrollment*. 
  • Two passport size photos. 
  • Health Record Form
  • Medical Physicians Form
  • A copy of the immunization records. 
  • MS/HS Recommendations Form.

All Applicants

The RM 1,200 non-refundable application fee per student application can be made through bank remittance (details here) or paid in person to Ms. Alice Tan, our School Fees Executive. For additional questions regarding application fee and/or school fees, please contact Ms. Alice Tan.

Once we have the required documentation on file and the RM 1,200 application fee has been paid, we will then move forwards to schedule admissions assessment accordingly.
